Before purchasing you must fill out this form and  or do a phone interview. all potential owners must have a phone interview.

Thanks for your interest in a Mavericks Kennel Puppy, we take this breed and new ownership very seriously.

Will this be an inside or outside dog? If outside what type of housing will be provided? Do you have a fence?

If you are leasing is your landord in agreement for you to have a dog and this type of dog? If so are you willing to provide proof from the landord?

If for any reason you were unable to keep the dog, would you agree to notify Mavericks Kennel ASAP ?
